Polychronis Tzortzakis (Greek: Πολυχρόνης Τζωρτζάκης; born 3 January 1989 in Chania) is a Greek road and track cyclist, who last rode for UCI Continental team Tarteletto–Isorex. Representing Greece, he competed in the team pursuit event at the 2010 and 2011 UCI Track Cycling World Championships.
